Post-Hospital Discharge Private GP Follow-Up
A steady clinical hand for the days and weeks after you leave hospital — at home, at your hotel or in clinic.
The transition from hospital back home is where many problems quietly appear: unclear medication changes, unresolved symptoms, missed follow-up, or simply the sense that no one is holding the overall picture.
A private GP with hospital medicine experience can bridge that gap. Dr Maytham Omran's background in acute hospital and general practice means discharge summaries are read carefully, medication is reconciled properly and follow-up is arranged coherently.
What a follow-up typically covers
- Careful review of the discharge summary and hospital investigations
- Medication reconciliation and safe adjustment where appropriate
- Clinical review of recovery, wounds, mobility and general wellbeing
- Coordination of outpatient specialist appointments and imaging
- Written communication with the discharging team and, where relevant, insurers
- Family-facing summary of what to look out for and when to seek help
Where we see you
If travel is difficult after discharge, we visit at home or at your hotel across central London. In-clinic reviews at Harley Street and secure video follow-ups are also available.
When to seek urgent help
A follow-up service is not a substitute for emergency care. New severe pain, breathlessness, bleeding, fever with confusion, or wound concerns that are rapidly worsening should be assessed urgently — call 999 or attend A&E if there is any doubt.
Common questions
How soon after discharge should the review happen?
For most patients, within the first few days after discharge is ideal. Same-day visits can be arranged where the clinical picture warrants it.
Can you work alongside my hospital consultant?
Yes. We routinely coordinate with hospital consultants and outpatient teams so that ongoing care runs smoothly.
Do you provide ongoing GP care afterwards?
Yes, either on a per-consultation basis or through a membership plan for continuing private GP care.
